Purpose of the Grant

Serve individuals eligible for services under the Transition to Community Initiative in accordance with Minnesota Statues, section 256.478, subdivision 2. COUNTY shall serve individual and provide services identified by COUNTY in consultation with the STATE’s Transition to Community Initiative Team. COUNTY shall complete and submit a referral to the STATE’s Transition to Community Initiative Team to request reimbursement for services and expenditures. COUNTY shall submit referrals in a form and format determined by STATE. STATE shall review the referral to determine whether request is approved. COUNTY shall participate in consultation with STATE and other stakeholders as needed and as requested by the STATE’s Transition to Community Initiative Team. All expenditures must be approved by STATE pursuant to Minn. Stat. §256B.092, subd. 13. STATE shall determine the expenditure amount to be reimbursed under this CONTRACT. STATE has the authority to approve or to deny approval of any and all services and expenditures.
