The purpose of the contract was to provide collaboration with the MDH BOLD Team to support Minnesota’s Centers for Disease Control and Prevention (CDC) BOLD Grant work plan, which aims to support the promotion of brain health among populations with a high risk of Alzheimer’s disease and related dementias. The Contractor will be tasked with strengthening partnerships with clinics and establishing referrals for supportive services, assessing the use of non-physician team members in care of people with dementia and identifying areas for improvement of care coordination, developing training for Community Health Workers (CHWs) and co-developing culturally responsive Alzheimer’s related messages.
