Summarize purpose of the contract, including why it was necessary: Provide 1.0 Full Time Employee (“FTE”) to work within the Governmental Unit’s Human Services and Public Health Department to assist former service members experiencing homelessness. The staff member(s) provided will work with former service members experiencing homelessness, assisting them in developing a transition plan, this plan should ensure the former service member secures permanent housing. These services were necessary to support the Minnesota Department of Veteran’s (MDVA) goal of ending Veteran homelessness.

Evaluate the contractor's timeliness 4: At the beginning of the contract there was a minor delay in submitting the monthly invoice, as staffing in the vendor’s accounting office increased the delay was remedied.
