The Minnesota Department of Health’s (MDH) Diabetes and Cardiovascular Health Programs seek to engage community partners in the development and implementation of the 2030 Cardiovascular and Diabetes State Plan to address primary prevention, acute treatment, and disease management strategies for these chronic conditions. MDH seeks independent consultants to develop a statewide Plan that reflects a wide variety of stakeholder input, deepened engagement of disproportionately impacted communities, and improved engagement between communities and MDH that strengthens relationships, trust, and investment in a shared or overlapping plan of action.

Collective Action Lab has been selected as a sole source contractor because they have a proven track record of engaging communities experiencing the greatest health disparities around complex issues that cut across clinical, community and government systems. In addition, consultants at the Collective Action Lab have deep knowledge and many years of experience planning and facilitating meetings between diverse stakeholders in order to build trust, identify common priorities and foster shared ownership. Finally, they have experiencing gathering and synthesizing input from diverse stakeholders to develop reports and tools to communicate a shared vision and identify tangible strategies that encourage stakeholders to take action.
