This contract related to improving management of chronic kidney disease (CKD). The work was nested in a clinical trial testing a CKD clinical decision support tool in the HealthPartners/Park Nicollet Clinical System. This contract supported development of patient education materials for CKD by HealthPartners Institute and MDH staff and integration of the patient education materials into the intervention arm of the clinical trial and the clinical system's electronic health record.

CKD is a chronic condition that is both underdiagnosed and insufficiently managed in clinical practice. This trial was important for identifying ways to improve clinical practice. The addition of patient education materials would explore the potential importance and impact of addressing any knowledge and communication barriers for patients with CKD. Results from the study would inform approaches for how to improve clinical care for CKD across Minnesota.

The contractor was on time and communicative about timeline changes due to the COVID-19 pandemic which disrupted clinical care as well as the trial.
The contractor offered access to a high quality clinical decision support tool and did a great job integrating the patient education materials into the tool. The contractor even worked with us to configure their tool to support translation of the patient education materials into their clinical decision support tool. The patient education materials would have benefitted by having broader engagement with other members of the HealthPartners Clinical team (e.g., primary care providers using the tool with patients, dieticians, nephrologists) contributing but this was EXTREMELY difficult during the COVID-19 pandemic and the challenges that health systems faced afterward with provider shortages and challenges with burnout.

The contractor performed well. They prioritized the work, engaged in regular meetings and provided updates, partnered with MDH in designing tools, and worked to build solutions that could be integrated for sustainability.
